ITMAV outreach work

The project
Created at the initiative of ITMAV (Initiatives de travail de milieu près des personnes aînées en situation de vulnerability) aims to provide an upstream response to the lack of awareness among seniors and their caregivers regarding existing resources and how to access them, as well as the lack of support for vulnerable seniors in finding solutions to their difficulties.
The project is being implemented in the six sectors of the local service network (RLS) of the Mont-Royal: Côte-des-Neiges, Outremont, Parc-Extension, Milton-Parc, Ville Mont-Royal, Downtown West and Westmount.

The Objective
The project aims to identify elderly people in vulnerable situations or at risk of becoming vulnerable and direct them to the appropriate services or activities, in collaboration with community resources. It helps foster their connection or reconnection with relevant community resources and supports them in finding solutions to the problems they encounter. Thus, it contributes to improving their well-being, realizing their autonomy, and thus maintaining their active participation in the community.
The Approach
- Present information workshops by the community worker
- Identify vulnerable older adults, understand their needs, and direct them to appropriate resources
- Establish an Age-Friendly Community in each community, through the collaboration of community partners.
